Immunonkologi är ett spännande område som på grund av utbredd läkemedelsresistens ännu inte nått sin fulla potential. 2cureX funktionella precisionsmedicinverktyg IndiTreat kan kringgå problemet genom att på förhand avgöra vilken behandlingsregim som fungerar bäst för varje enskild patient. Under veckan erhöll bolaget ett bidrag från det tyska utbildnings- och forskningsministeriet för att anpassa IndiTreat till att hitta bästa tillgängliga immunterapi för patienter med äggstockscancer. Projektet bedrivs i samarbete med University Medical Center Hamburg-Eppendorf. BioStock kontaktade med 2cureX vd Ole Thastrup för en kommentar.
2cureX är kända för sin patenterade IndiTreat-teknik – ett test som används för att matcha en enskild cancerpatient med den mest effektiva behandlingen för just den individen. Tekniken ligger i framkant av det allt hetare fältet Funktionell PrecisionsMedicin (FPM), ett fält som forskningen antyder är ett användbart komplement till den mer klassiska, men mindre tydliga, formen av precisionsmedicin, Genomic Precision Medicine (GPM).

IndiTreat ger immunonkologin ny luft under vingarna

Att traditionella cancerbehandlingar som kemoterapi och strålning inte alltid når optimalt behandlingsutfall har öppnat upp fältet för det nya terapeutiska området immunonkologi (IO). IO går i stora drag ut på att ta fram antikroppar eller andra typer av immunmolekyler som kan motverka de skadliga effekterna av cancerceller. Många studier har visat att IO-terapi kan vara mycket effektiv, inte minst vid individanpassad behandling, eftersom den kan hjälpa patientens immunsystem att attackera cancertumörer på ett effektivare sätt.
Enligt en ny marknadsrapport förväntas den globala IO-marknaden vara värd över 157 miljarder USD år 2027. Såväl läkemedels- och bioteknikbolag har satsat stort på IO och dess möjlighet att förbättra behandlingen av cancerpatienter, men trots tydliga studieresultat har framsteg i termer av verkliga behandlingsutfall låtit vänta på sig. Det är här IndiTreat kan göra skillnad. FPM-testet besitter nämligen förmågan att förutsäga vilken specifik IO-behandling, eller kombination av IO-behandlingar, som passar bäst för varje enskild patient.
Hittills har man kunnat visa att IndiTreat-testet inte bara minska belastningen på patienter, som annars skulle kunna drabbas av biverkningar av onödiga läkemedel eller utveckla läkemedelsresistens, utan även minskar trycket på läkare som fått förlika sig med det faktum att inte alla behandlingar fungerar likadant på varje patient.

2cureX har äggstockscancer på radarn

För närvarande genomgår IndiTreat klinisk validering för användning vid metastaserande kolorektalcancer, och en fullvärdig lansering av FPM-testet inom denna indikation planeras under andra halvan i år. 2cureX är dock övertygade om att testet kan fungera väl även inom andra cancerindikationer.
En sådan indikation är äggstockscancer som enbart i Europa berövar mer än 40 000 kvinnor livet varje år, i många fall till följd av bristen på effektiva och långtidsverkande behandlingsregimer. Läkemedelsresistens är ett vanligt fenomen bland patienter med äggstockscancer. Nästan alla patienter utvecklar resistens mot sin första behandling och mer än hälften dör inom fem år efter diagnos.
Idag finns inga godkända IO-terapier för behandling av äggstockscancer, men flera kandidater befinner sig i sena kliniska studier. Med beprövad FPM-teknik som IndiTreat, som kan förutspå effekten av både existerande och nya IO-terapier som monoterapier eller i kombination med andra cancerläkemedel, skulle det prognosticerade utfallet för äggstockscancer kunna förbättras betydligt. Detta genom att förutsäga hur varje patient kommer att reagera på respektive läkemedelsalternativ och därmed bidra till att påskynda utvecklingen av nya IO-terapier.

Tyska myndigheter ser stor potential i IndiTreat

Det tyska utbildnings- och forskningsministeriet (Bundesministerium für Bildung und Forschung, BMBF) var tidiga med att se potentialen hos IndiTreat. Redan 2017 tilldelade man 2cureX tyska dotterbolag, 2cureX GmbH, och dess kliniska samarbetspartner universitetssjukhuset Hamburg-Eppendorf (UKE) 810,216 euro för ett projekt som utvärderade IndiTreat hos patienter med äggstockscancer.
Efter att projektet uppvisat goda framsteg beslutade BMBF tidigare i veckan att tilldela det ytterligare bidrag om 1 322 520 euro, varav 553,280 euro går direkt till 2cureX GmbH. Bidraget är avsett att skapa förutsättningar för att IndiTreat ska komma patienterna till gagn så snart som möjligt.

Ole Thastrup, as IndiTreat moves closer to full commercialization for use in colorectal cancer patients by being clinically validated in hospitals across Europe, how is this project in ovarian cancer patients different?
– The ovarian cancer project is in earlier stages compared to the colorectal cancer project, meaning that commercialization of an IndiTreat ovarian cancer test will happen subsequent to the one for colorectal cancer.
– The ovarian cancer endeavour is supported by active clinical collaborations with Rigshospitalet in Copenhagen and our long-term partner UKE in Hamburg. In Hamburg, we concentrate on developing an IndiTreat test that can measure the direct interaction between the patient’s immune-system and the patient’s tumour. Such test will address the fastest growing market segment within oncology: immuno-oncology.
As a follow-up, how confident are you that IndiTreat will work in ovarian cancer, as it does in colorectal cancer?
– As a young innovative company, we are obviously convinced that it will work, but you will get a more neutral judgement from the involved, highly recognized hospitals. They will only spend resources and credibility in such project, if they really believe in it.
For what stage(s) of ovarian cancer would IndiTreat be most suitable?
– We are concentrating on ovarian cancer patients in the later stages (III and IV).
The project in ovarian cancer is focused on Immuno-oncology and predicting which IO therapies would be most effective in patients. How would this work even though no known IO therapies have yet been approved for treating ovarian cancer?
– You can say that we are out early; but it is globally expected that some of the large number of ongoing clinical trials with IO drugs in late stage ovarian cancer will turn out positive. 2cureX want to be ready when that happens with an indispensable tool for stratifying those patients into responders and non-responders.
One of the goals with this project to help speed up the clinical studies with new IO therapies. How much of an impact do you believe IndiTreat could have here?

Ole Thastrup, vd 2cureX

– There is a clear trend in clinical trial execution to include biomarker tools for selecting drug-sensitive patients before they are enrolled in specific clinical trials. IndiTreat IO will be uniquely positioned to provide such stratification within immuno-oncology. An effective stratification will save resources and time, thus help in getting successful drugs faster to the market.
The collaboration with UKE began some years back. Has it been as fruitful as you had expected so far and what are your expectations moving forward?
– Indeed, we have had a long-term collaborative interaction with UKE. UKE is highly recognized internationally within all aspects of cancer treatment. Initially, the collaboration was scientifically driven; now the collaboration has shifted to provide clinical benefits to the patients.
How important is this newly received grant for the ovarian cancer project, and how far do you believe it will bring you?

You are currently running an Early-Access Program with IndiTreat across European clinics so that it can already be used in colorectal cancer patients. Can we expect a similar program down the line for ovarian cancer?
– We will have early access activities also for ovarian cancer; but it will not be constructed exactly as the current one for colorectal cancer, as there is a number of aspects of the current program that can be used directly in the subsequent commercialization of the IndiTreat IO test.
Will the current commercialization process affect the ovarian cancer project at all, and, if so, how?
– It will. The current commercialization process is providing 2cureX organization readiness to commercialize. This includes, for example, logistics, reimbursement/market access practices and clinical guidelines. Information that will be applicable also to the ovarian cancer project.
Finally, on a more general note, we know pancreatic cancer is another indication IndiTreat is expanding to. Do you expect further expansion to more cancer indications later on?
– We are receiving requests from clinical institutions for going into other solid cancers. For now, our focus remains on moving IndiTreat to the market for colorectal, ovarian and pancreatic cancers.
